Technique of Tissue-preserving, Minimally-invasive Total Hip Arthroplasty Using a Superior Capsulotomy

چکیده

Preservation of the tissues surrounding the hip may improve hip joint stability and facilitate recovery. A new technique for performing total hip arthroplasty with a superior capsulotomy allows for maximal preservation of the hip joint capsule and surrounding muscles. Using this technique, the gluteus medius and minimus are reflected anteriorly and the piriformis is reflected posteriorly. The short external rotators and posterior capsule are left intact. In most cases, the femur is instrumented with straight instruments before the femoral head is excised. This provides additional strength and stability to the femur during preparation. The femoral head is excised, rather than dislocated, to minimize disruption of surrounding tissues. The acetabulum is prepared and the acetabular component is inserted with angled instruments. This allows the femur to remain in physiologic positions throughout the procedure. Experience with this procedure demonstrates that, while technically demanding, the patients return to a normal gait more rapidly, with unrestricted motion and weight bearing postoperatively. The procedure offers the opportunity to facilitate recovery while potentially decreasing both shortand long-term surgical complication rates compared with conventional total hip arthroplasty techniques.

Tissue-Preserving, Minimally Invasive Total Hip Arthroplasty Using a Superior Capsulotomy

Conventional total hip arthroplasty in its many forms has been well established as a reliable procedure with predictable recovery. The high volume of procedures has allowed the incidences of the most common post-operative complications to be determined. Early complications vary widely by surgical approach and method of fixation, but generally include infection, dislocation, abductor morbidity, ...

Use of the Fitmore® hip stem bone-preserving system for the minimally invasive anterior-supine approach in hip replacement.

Total hip arthroplasty through a single-incision anterior approach is a minimally invasive surgical (MIS) technique that allows component placement without violation of the posterior hip capsule or "hip deltoid." This allows faster recovery without dislocation precautions. The Fitmore® hip stem (Zimmer, Warsaw, Ind) is a bone-conserving stem designed for use in MIS techniques. The technique des...

Little Clinical Advantage of Muscle Sparing over Partial Resection of Gluteus Muscles in Minimally Invasive Anterolateral Total

Introduction Minimally invasive total hip arthroplasty (THA) has become common, and several approaches are now used clinically. We have been using an anterolateral (AL) approach with partial resection of the gluteus medius in minimally invasive THA. Recently, faster recovery has been reported using the AL approach with muscle sparing technique (modified Watson-Jones approach) [1]. Our concern i...

Cost effectiveness of minimally invasive total hip arthroplasty

CRD summary The objective was to examine the cost-effectiveness of two minimally invasive techniques for total hip arthroplasty, which were the mini-incision technique and the Minimally Invasive Solutions two-incision hip procedure. These were compared with conventional surgery. The authors concluded that the minimally invasive techniques were more effective and less expensive than the conventi...

THA performed using conventional and navigated tissue-preserving techniques.

Less invasive methods of performing total hip arthroplasty have been considered controversial after increased complication rates and component malpositioning were reported. A new method of performing total hip arthroplasty through an incision in the superior capsule, posterior to the abductors and anterior to the posterior capsule, was developed with the aim of producing a technique that mainta...
